Appointment requests

The appointment form collects the contact and scheduling information you choose to provide. It sends an administrative notice to info@innovationdrivedental.com and stores the request in the private Admin inbox so the office can respond. Sending a request does not create or confirm an appointment.

Secure patient forms

New-patient, insurance, and records-release forms are submitted to private server storage. Form answers are encrypted at rest. The office notification email contains a form type, patient name, and reference number—not medical or insurance answers. The authenticated Admin inbox is used to view the submission.

Live chat

Live chat stores the name and messages entered during a website conversation. Do not use chat for medical histories, insurance numbers, payment details, emergencies, or other sensitive information.

Website analytics

The site may record page views, general click targets, and scroll depth to understand how pages are used. It does not intentionally record form-field contents, camera data, or literal eye movement. Browser “Do Not Track” signals are honoured by the first-party analytics endpoint.

Service providers and retention

The website is hosted on DreamHost. Information is retained only as needed for the office workflow, security, legal obligations, and continuity of care. Access is limited to authorized practice personnel. Internet transmission can never be guaranteed risk-free.

Your choices

You may call the office instead of using a website form. To ask about website information, correction, or deletion, contact info@innovationdrivedental.com or call 905.264.0333. Clinical-record obligations may limit deletion of information that has become part of a patient record.
